Pudendal Neuralgia

It is a chronic neuropathic pain syndrome that severely impairs quality of life, arising from the damage, entrapment, or irritation of the pudendal nerve, one of the most important nerves of the pelvic floor. Its most characteristic feature is pelviperineal pain that is exacerbated by sitting and relieved by standing or lying down.

What is Pudendal Neuralgia?

The pudendal nerve is a critical peripheral nerve located within the pelvic floor. It provides both sensory innervation to the external genitalia, anus, and perineum, and motor control to the crucial muscles of this region.

The nerve originates from the sacral region of the spinal cord (S2-S4) and courses through narrow anatomical pathways (the Alcock canal) between the muscles and ligaments of the pelvic floor - specifically the sacrospinous and sacrotuberous ligaments. Pudendal neuralgia develops as a result of damage, entrapment, or irritation of the nerve along this course.

Common Symptoms

Nature of the Pain
Described as burning, stinging, electric shock-like sensations, stabbing, or paresthesia (tingling/numbness). Symptoms can be unilateral or bilateral.
Pain Distribution
Pain is localized to the perineum (the area between the anus and external genitalia), anus, vulva (in females), or scrotum and penis (in males). Patients frequently describe a foreign body sensation, as if sitting on a tennis ball or a stone.
Pain Exacerbated by Sitting
Classically, the pain worsens with sitting and is relieved by standing or lying down.
Urinary and Defecatory Dysfunction
A persistent sensation of urinary urgency, dysuria (burning during urination), urinary frequency, a feeling of urinary incontinence, tenesmus (straining during defecation), or a constant sensation of rectal fullness.
Sexual Dysfunction
Severe pain during intercourse (dyspareunia), genital hyperalgesia/allodynia (where even a light touch that normally does not cause pain triggers severe pain), or persistent sexual arousal syndrome / loss of orgasm.

Diagnosis Methods

Detailed Medical History
The character, location, and especially the relationship of the pain to sitting are questioned in detail.
Physical Examination
Sensory assessment of the perineal region and examination of the pelvic floor muscles are performed.
Pelvic 3 Tesla MRI and Pudendal MR Neurography
High-resolution imaging is used to evaluate the nerve pathway and areas of entrapment.
Pudendal Nerve EMG in Sitting Position
Electromyography performed in the sitting position detects conduction abnormalities in the nerve.
Pudendal Nerve Block
A diagnostic block is a powerful method for confirming that the pain originates from the pudendal nerve.

Causes

Mechanical Entrapment (Compression)
Entrapment of the nerve within the Alcock canal or due to the hypertrophy/thickening of the surrounding ligaments (Pudendal Nerve Entrapment - PNE).
Chronic Microtrauma
Prolonged and repetitive cycling, horseback riding, or sitting on hard surfaces for extended periods.
Surgical and Medical Interventions
Stretching of the nerve or its entrapment within suture/scar tissue during pelvic surgeries.
Birth Trauma
Excessive stretching or compression of the nerve during difficult or prolonged vaginal deliveries.
Previous Injuries
Severe blunt trauma or falls onto the pelvic region or buttocks (e.g., falling directly onto the tailbone).
Pelvic Floor Muscle Spasms
Chronic hypertonia (excessive contraction) of the pelvic floor muscles, which increases mechanical pressure on the nerve.

Treatments

Lifestyle Modifications and Conservative Management
Specialized cushions with a central opening (U-shaped or donut-shaped) are used to reduce direct pressure on the pudendal nerve while sitting; cycling and prolonged, uninterrupted sitting are avoided.
Pelvic Floor Physical Therapy
Specialized internal and external manual therapies performed by dedicated pelvic floor physical therapists aimed at resolving muscle spasms, releasing myofascial trigger points, and achieving nerve mobilization.
Pharmacotherapy
Conventional analgesics are typically ineffective against neuropathic pain. Therefore, anticonvulsants (e.g., gabapentin, pregabalin) or tricyclic antidepressants (e.g., amitriptyline) are preferred.
Pudendal Nerve Block (Interventional)
Infiltration of a local anesthetic and corticosteroid mixture into the precise anatomical entrapment sites under image guidance (ultrasound or fluoroscopy). It is a highly potent tool for both diagnostic confirmation and therapeutic pain relief.
Pulsed Radiofrequency (PRF) Modulation
A modality utilized in patients who experience significant but short-lived relief from nerve blocks, aimed at modulating pain signaling without causing histological damage to the nerve.
Robotic Decompression of the Pudendal Nerve
In resistant cases that do not respond to conservative and interventional methods, surgical decompression is performed to eliminate the pressure on the nerve.

The nature of the pain is the greatest clue to its source. Tingling and electric shocks indicate neuropathic pain, while aching and throbbing point to nociceptive pain.
