It is a chronic neuropathic pain syndrome that severely impairs quality of life, arising from the damage, entrapment, or irritation of the pudendal nerve, one of the most important nerves of the pelvic floor. Its most characteristic feature is pelviperineal pain that is exacerbated by sitting and relieved by standing or lying down.

The pudendal nerve is a critical peripheral nerve located within the pelvic floor. It provides both sensory innervation to the external genitalia, anus, and perineum, and motor control to the crucial muscles of this region.The nerve originates from the sacral region of the spinal cord (S2-S4) and courses through narrow anatomical pathways (the Alcock canal) between the muscles and ligaments of the pelvic floor - specifically the sacrospinous and sacrotuberous ligaments. Pudendal neuralgia develops as a result of damage, entrapment, or irritation of the nerve along this course.
